Lung Health
The Lung Health Program is a multi-disciplinary program, aimed at reducing Chronic Obstructive Pulmonary Disease (COPD) exacerbations and optimizing care for individuals living with COPD. The program consists of the following:

Identification of patients who are suspected of having COPD

Confirmation of COPD diagnoses with spirometry

Counselling with a pharmacist and nurse practitioner to review medications, develop a COPD action plan and appropriate follow-up

Case consultation with a respirologist, as required

Group educational sessions (in development)
The objective of the program is to empower patients to develop self-management skills and tools to help better manage their COPD.
Suitable for: individuals older than 18 with suspected COPD.
