Board of Directors and Officers

Dr. Christopher Gilchrist

Dr. Chris Gilchrist is a family physician with the Trillium-Summerville Family Medicine Teaching Unit. He also serves locally as a palliative care physician with the Dorothy Ley Hospice Community Physicians. Dr. Gilchrist is in his fifth year on the Board of Directors and currently in his first year as the Chair. He has worked within Summerville to build palliative care supports by collaboratively developing a clinic for early palliative care and creating a home visiting palliative care program with the family medicine residents. Academically, he is faculty with the University of Toronto and serves as the Associate Director of Clinical Skills for the MD Program and locally as the Academic Half Day Coordinator for the family medicine residents.
Dr. Angela Lalla

Dr. Angela Lalla is a family physician who has been practicing at the Etobicoke site of Summerville Family Health Team since 2014. She completed medical school at the University of Ottawa and her family medicine training at McMaster University. She is a Lecturer with the Department of Family and Community Medicine at the University of Toronto and teaches clerks and family medicine residents. Dr. Lalla is in her fourth year on the Board of Directors and currently in her third year as Vice Chair.​

Dr. Ruth Connelly

Dr. Ruth Connelly is a family physician practicing at the Harborn site of the Summerville Family Health team since its inception. She has practiced in Mississauga since 1985, worked for the Peel Region Health Department in their Healthy Sexuality clinic, and is a Lecturer with the University of Toronto Medical school, teaching clerks and family practice residents. She has a special interest in the field of mental health.  Dr. Connelly is now in her 6th and final year on the Board, having been Chair for the last 3 years.

Dr. Mervyn Meharchand
Susannah Margison

Susannah Margison is a medical malpractice defence lawyer whose practice focuses on medical malpractice litigation on behalf of health professionals, hospitals and nursing homes. Susannah received an honours baccalaureate in Health Sciences from the University of Ottawa and went on to attend the Schulich School of Law at Dalhousie University where she specialized in Health Law and Policy.  She was called to the bar in 2012. Susannah has been a community member of the SFHT Board since 2018 and she is the chair of the Risk Management Subcommittee. In 2021, Susannah founded Susannah Margison Coaching, a service which specializes in executive, life and negotiation coaching.

Dr. Vincent Park

Dr. Vincent Park is a family physician with the Trillium-Summerville Family Team at Central Site.  He is one of the founding members of the Summerville Family Health Team that was formed in 2007.  Dr. Park is in his third year on the Board of Directors.  He is also a member of the IM committee at the Summerville Family Health Team.  Academically, he is on faculty with the University of Toronto as an adjunct lecturer, involved in clinical teaching of third year medical students.​

Adeela Yaqub

Adeela Yaqub is an experienced data and analytics specialist with over 17 years of professional experience with 10+ years of healthcare experience in strategy development, data management, transformative analysis, and change management. Adeela is a Group Manager in Ontario Health practice specializing in the design, development, and implementation of data strategy including data governance, operating models, and data driven culture. Adeela has been a community member of the SFHT Board since 2019 and as well she is the chair of the Dashboard Subcommittee. Furthermore, Adeela successfully obtained PMP, Lean Six Sigma Black Belt and Certified Agile Project Manager certifications. Adeela is enrolled in an Executive Master of Business Administration (EMBA) program (University of Fredericton) with a specialty stream in Business Analytics Leadership. 
Dr. Al MacKintosh

Dr. Alasdair Mackintosh grew up in Alberta and graduated from the University of Alberta (M.D., 1984). He came to Ontario for his Family Practice residency at the University of Western Ontario. After additional training in Obstetrics, he spent several years working in Emergency departments in Woodstock & St. Thomas, and doing Family Practice locums in Toronto and other southern Ontario locations.

Following an exciting year of world travel, he decided to make his home in Ontario, and joined Apple Hills Medical Associates in July 1989. In the years since then, his Family Practice has included delivering many babies, caring for people of all ages, with a special interest in palliative care. Dr. Mackintosh was one of the original physicians in the start up of the Palliative Care Unit at Trillium Hospital and also worked in the ER at the Queensway and Mississauga Hospitals (now known as Trillium Health Partners). Although he no longer does obstetrics or Emergency Dept. work, he continues to do hospital and home visits to his patients who are unable to come to his office.

In 2007, AppleHills Medical became part of Summerville Family Health Team, and since then Dr. Mackintosh has served as both a board member and Board Chair, and is currently the Lead Physician for Summerville FHO Dr. Mackintosh is married and has 2 daughters, enjoys reading, skiing and participating in a Masters swimming program.

Andrea Stevens

Andrea Stevens, MBA, has been the Executive Director of Summerville Family Health Team since June 2018.  Throughout her career, she has held senior management positions in the health and non-profit sectors, including 7 years as Vice President at the Ontario Lung Association, 2 years as the inaugural Executive Director of the Etobicoke Medical Centre Family Health Team, and more than 25 years at the Centre for Addiction and Mental Health. In addition, she has done private consulting for the Government of Ontario, Health Canada, the Canadian Mental Health Commission, the University of Toronto and other organizations specializing in mental health promotion, substance use prevention, and management. She has co-authored several publications and is a member of CIHR’s College of Reviewers.
