Summerville Family Health Team Clinical Pharmacists work directly with your family doctor and the rest of your health care team to ensure the most appropriate use of medications.

Make an appointment to see the Clinical Pharmacist if you:

  • Want to learn more about your medications, including when to take them, potential side-effects and drug interactions
  • Have been recently discharged from the hospital with changes to your medications
  • Have questions about over-the-counter, herbal or natural health products
  • Would like to optimize your medications for treatment of chronic conditions such as diabetes, cardiovascular diseases, mental health, chronic pain, and respiratory diseases.

When you meet with the Clinical Pharmacist, he/she will review your medications and work with you and your doctor to help you meet your medication-related goals. Please have all prescription medications, over-the-counter medications, vitamins and herbal products that you use available for your appointment with the Clinical Pharmacist.
